Are two shocks better than one? Double sequential defibrillation for refractory ventricular fibrillation
Many out-of-hospital cardiac arrest related deaths are attributed to ventricular fibrillation or pulseless ventricular tachycardia (VF/VT). Despite advances in resuscitation care, patients may remain in VF/VT after multiple standard defibrillation attempts. Double sequential external defibrillation has been studied for patients in both refractory atrial fibrillation and refractory ventricular fibrillation. The goal of this study was to determine the feasibility and safety of a cluster randomized controlled trial with crossover comparing vector change defibrillation or double sequential external defibrillation to standard defibrillation for patients experiencing refractory ventricular fibrillation.
